I too had the same issue, for my Hori FS3 Wireless, the stick works only in the menu but fails when in the game itself.
I solved this by downloading by Xbox360ce. It made my arcade stick discoverable and edit my stick buttons. I was able to access the MAME settings while running Core Type R MAME. I changed the controls from there.
